Rule (15A NCAC 18A .2662 GRADING)
(a) The grading of food establishments is based on a system of scoring. A food establishment that earns a score of at least:
(1) 90 percent shall receive a grade A;
(2) 80 percent and less than 90 percent shall receive a grade B;
(3) 70 percent and less than 80 percent shall receive a grade C.
Permits shall be immediately revoked in accordance with G.S. 130A-23(d) for food establishments receiving a score of less than 70 percent.

Corrected During Inspection - PIC immediately replaced paper towels."Summary: The handwashing sink was inaccessible due to improper storage.
5-205.11 Using a Handwashing Sink - Operation and Maintenance (Pf):
"Handwashing sinks closest to cook line was not accessible due to hose being stored around faucets and in basin as well as large piece of equipment stored underneath handwashing sink. A handwashing sink shall be maintained so that it is accessible at all times for employee use. Corrected During Inspection - Advised PIC to remove equipment from handwashing sink to ensure it is accessible at all times."Summary: Raw chicken was not cooled properly, exceeding safe temperatures.
3-501.14 Cooling (P):
"Several containers of raw chicken that were seasoned around 8:28 a.m. today were observed between 45-58°F in walk-in cooler. Cooling shall be accomplished by cooling food within 4 hours to 5°C (41°F) or less if prepared from ingredients at ambient temperature. Corrected During Inspection - Inspector discussed ways to speed up the cooling process after prep, such as using colder water, adding ice, utilizing smaller food storage containers with lids off, or possibly utilizing metal pans which facilitate heat transfer, etc. PIC immediately pulled this product to be placed into walk-in freezer to quickly cool them down to temperature."Summary: Food was stored directly on the floor, violating food storage protocols.
3-305.11 Food Storage - Preventing Contamination from the Premises (C):
"Food items stored directly on floor inside walk-in freezer, in back prep area, as well as near storage shelf between cook line and prep line. Food shall be stored at least 6 inches off of floor."Summary: Employees were not wearing adequate hair restraints.
2-402.11 Effectiveness - Hair Restraints (C):
"Employees observed without adequate hair restraints upon arrival for inspection. Use head coverings, beard guards and clothing to restrain body hair from contacting exposed food, equipment, and utensils."

3-501.16 (A) (2) and (B) Time / Temperature Control for Safety Food, Cold Holding (P):
"All temperature control for safety foods, such as lettuce, tomato, cheeses, sour cream, half and half, inside make unit at prep line were observed between 43-45°F. Temperature control for safety foods shall be maintained at 41°F or less. Corrected During Inspection - PIC supplied ice to product upon discovery. Inspector advised PIC to adjust the temperature of the unit to lower setting and have unit assessed to ensure unit is maintaining internal food temperatures at 41°F or less at all times."Summary: Chemical spray bottles were improperly stored near food and clean dishes.
7-201.11 Separation - Storage (P):
"Chemical spray bottle stored on drainboard of three-compartment sink, next to sanitizing compartment, used for storing clean dishes. Chemical spray bottle stored directly next to plastic wrap in back prep area. Store toxic materials to avoid contamination of food, equipment, utensils, linens, and single-service/use items. Corrected During Inspection - PIC immediately relocated these items to proper chemical storage area."Summary: Sanitizer concentration in the bucket was insufficient.
3-304.14(B):
"Sanitizer bucket at cook line observed at 0 ppm (quaternary ammonia). Hold in-use wiping cloths in properly concentrated sanitizer between uses. Corrected During Inspection - PIC immediately discarded contents and refilled with sanitizer at proper concentration."Summary: Detached pipes under the ice machine caused water to pool on the floor.
5-205.15:
"Pipes underneath ice machine are detached, therefore water draining out of machine is causing standing water on floors, as water is not going into floor drain. Plumbing fixtures shall be maintained in good repair."Summary: Cardboard boxes were left in the dumpster enclosure, creating debris.
5-501.115:
"Large accumulation of cardboard boxes on ground of dumpster enclosure. Storage areas and enclosures for refuse shall be maintained free of unnecessary items and kept clean."
